The following form is designed to help draft your trust agreement. You are not required to answer any questions after the first three general questions. You may skip any questions you are unsure of or would like additional consultation about. Please answer the questions as they relate to your current circumstances. We will discuss timing implications during your consultation.

Each spouse should complete this questionnaire if the trust is being funded by a married couple. We will reconcile any answers that differ and will note any differences during the consultation. You will have time to change any answers before the trust is finalized.

After you submit the agreement, you will be prompted to schedule a time for your consultation. We will provide you with a PDF draft of your trust agreement within 48 hours of your consultation for your review and comments.
