The following form is designed to help draft your trust agreement. You are not required to answer any questions after the first five general questions. You may skip any questions you are unsure of or would like additional consultation about. Please answer the questions as they relate to your current circumstances. We will discuss timing implications during your consultation.

After you submit the agreement, you will be prompted to schedule a time for your consultation. We will provide you with a PDF draft of your trust agreement within 48 hours of your consultation for your review and comments.
